Yes. USCIS requires that all adults 18 and older are up to date on certain vaccines at the time of their medical exam.
For ages 18 and older, USCIS requires the following (age-appropriate only): Tdap/Td (booster every 10 years), MMR (at least 1 documented dose or proof of immunity), Varicella (vaccine or reliable history of prior illness), Hepatitis B (1 documented dose if not previously immunized), Influenza (only if your exam is between Oct 1 and Mar 31), Pneumococcal (required starting at age 65 or earlier if medically indicated), COVID-19 (no longer required as of January 20, 2025), and Polio (effective May 1, 2024, all applicants over 2 months must show proof of vaccination).
One documented dose is usually enough for USCIS compliance, even if the vaccine is normally a multi-dose series (Hep B, MMR, Varicella). You do not need to finish the entire series before your I-693 is signed. Just the first dose and documentation that further doses are pending is sufficient.
You can either get the required vaccines at the time of your exam, or complete blood tests (titers) to check for existing immunity to MMR, Hepatitis B, and Varicella.
A reliable history of chickenpox illness is accepted by USCIS. No vaccine or blood test is needed if you have a documented history of the illness.
Some vaccines such as MMR and Varicella cannot be given during pregnancy. The civil surgeon will mark them as "contraindicated," and you will not need them to complete the I-693 at this time.
If you refuse a required vaccine without a valid medical reason, your exam will be marked "incomplete" and USCIS may deny your green card application.
After completing your immigration exam and receiving any required vaccinations, your provider completes all necessary paperwork and seals your results in an envelope. You then submit the sealed envelope directly to USCIS. Do not open the envelope unless specifically instructed by USCIS.
